By signing the adoption contract below: The Charlie Project Dog Rescue and the adopter(s) agree to all terms and stipulations including but not limited to Spay /Neuter and serves to protect the integrity of our mission statement.

Terms & Conditions*
- I acknowledge that the first few days after adoption may be stressful for my new dog. I agree to monitor my dog's eating and drinking habits during this time. If I have any concerns, I will contact a representative of The Charlie Project Dog Rescue immediately.
- I understand that puppies/dogs tend to have common health issues, such as coccidia, giardia, kennel cough/ worms/parasites. The Charlie project dog rescue has done its best to treat any ailments this dog may/may not have. I understand that rescue dogs are under stress which may cause them to break out with common ailments in the first few weeks. Although these conditions are treatable, they may require multiple treatments. I also understand that The Charlie Project Dog Rescue will not be responsible for any additional treatments once the dog is adopted.
- I understand that a vet visit is recommended within the first week of adoption and The Charlie Project Dog Rescue WILL REFUND my full adoption fee within the 7 days if the animal is not a good match or deemed unhealthy as long as I have followed guidelines and not deprived the dog of any measure.
- I acknowledge that I will not resell ,rehome or surrender this dog to anyone at any point in the dog's lifetime. The dog must stay with the original signer or designated person specified on the application. I understand that if I am unable to care for this pet, I will contact the Charlie Project Dog Rescue (TCPDR). I understand that if TCPDR is unable to find a foster, I shall be the foster. In that event, TCPDR will post the dog on Petfinder and other outlets, and TCPDR must approve the new home before placing the dog. No refund will be given at this time.
- I understand that a newly adopted animal may take weeks to adjust to its new home and family. This is why I have stated in my application that I am committed to providing the necessary training to address any issues that may arise. I am prepared to put in the time and effort to help the animal adjust to its new home and family. I am also prepared to provide a loving and supportive home for the animal.
- I understand that I am responsible for the care of my dog and that I must contact The Charlie Project Dog Rescue immediately if my dog is lost/ stolen, I will provide TCPDR with all relevant information about the dog's disappearance, including a description /picture of the dog, date /time of the disappearance, and the location where the dog was last seen. I will also provide TCPDR with my contact information, including my name, address, and phone number.
- I agree that the sole signatory of this contract is the only person who may have possession of the animal. The dog cannot be a gift, and if they are found to be residing at an address other than the original signer's residence, they may be removed.
- By signing this contract, I agree that I will spay/neuter this dog by 6 months of age. I acknowledge The Charlie Project Dog Rescue provided me ta list of vets approved to provide spay/neuter included with adoption fee. If I choose my vet or clinic TCPDR will reimburse me $100.00 towards the vet fee once proof of s/n is sent to thecharlieprojectdogrescue@gmail.com. If I do not comply I realize I will therefore forfeit any/all rights to the dog.
- I agree to keep the dog/puppy housed in my home as a family pet, and will sleep indoors at night. He/she will not be restricted to the yard nor tied there. OUR POLICY IS TO PLACE DOGS AS FAMILY PETS ONLY! Dogs adopted from TCPDR will not be allowed to be used for guarding/laboratory, illegal activity.
- I will not deprive my dog from proper nutrition, access to fresh food /water. I will not deprive my dog from any vet care/ vaccines, medications needed. I will provide heartworm /flea & tick medication monthly as needed.
- I agree by signing this contract to hereby release or waive any rights or claims against The Charlie Project Dog Rescue and their representatives/ volunteers, and/or foster/foster homes of any liability or damages to persons or property now or in the future caused by this animal.
- By signing this contract, I acknowledge that I release The Charlie Project Rescue and its Board of Directors, other volunteers, employees, and affiliates from any and all liability for any accident, injury, or health problem that may arise in connection with the rescue, care, and placement of any animal. I also acknowledge that I have read and understand the terms of this contract and that I have had the opportunity to ask questions about the contract and receive answers to my satisfaction.
- I agree that by violating any part of this contract may result in forfeiture of the adopted dog. TCPDR will communicate to work out any issues that arise for the dogs sake. If communication's break down and can not be resolved between both parties in this contract, TCPDR may reserve the right to resort to legal actions. All legal fees and court cost will be the responsibility of the signee.
